Assessment audit log
Track every change made to an assessment — including who made it, when it was made, and what was changed. The audit log provides a complete history of assessment updates, including content edits, structural changes, and user actions.
Where to find the activity log
To open the audit log:

Open the relevant assessment.

Click ⋯ (More actions) in the top right corner.

Select View activity log.

The audit log opens in a panel on the right side of the screen.
What’s shown in the audit log
Each entry in the audit log includes:

Date
-
Person or source (user, collaborator, agent, API key, automation, etc.)
-
Details of the change
Entries are grouped by day and sorted from most recent to oldest.

Types of changes recorded
There are three categories of changes recorded in the audit log:
1. Assessment actions
These are high-level actions performed on the assessment.
Examples include:

Assessment created

Collaborator invited
-
Collaborator submitted
-
Marked as draft
-
Marked as completed
-
Started editing
-
Stopped editing
-
Created clone
-
Exported
-
Set due date
-
Set label
-
Set owner
-
Restored version
-
Data types and location merged to inventory

Left comment (includes link to the comment)

Resolved comment (includes link to the comment)
If a version is restored and a question no longer exists, the log will display “Deleted question”, similar to version history.
2. Content changes (value updates)
These reflect edits to assessment answers and structured data.
For the initial phase, the audit log includes the following content updates:

When hovering over a content change, a tooltip displays the question number and question title.
3. Template updates (structure changes)
These reflect changes to the assessment structure:

Sorting and grouping
The audit log is sorted by date (most recent first).
Within each day:

Changes are grouped by editing session (save/version).
-
The time shown reflects the last change in that session.

Changes within the session are sorted by time.

If more than 7 changes occur in a session, older entries are collapsed under “Show more…”.
You can load additional days using Load more, when available.
Filtering the audit log
You can filter activity to focus on specific contributors.
Filter by person

Click the filter icon at the top of the activity log.

Select one or more names or sources.
-
Click Apply.
The filter includes any person or integration source that appears in the audit log (users, collaborators, TIA agent, API keys, automations, etc.).

Filter by question
To view activity related to a specific question:

Open the relevant question.
-
Click the ⋯ (More actions) menu on the question.

Select the activity option.
Filtering by question is only accessible from the question-level menu.
Version control behavior
-
Restoring a previous version does not delete or modify historical audit log entries.
-
Cloning an assessment does not copy the audit log. The cloned assessment starts with a new, empty log.

Changing versions does not retroactively alter past audit entries.
Permissions
-
Collaborators do not have access to the audit log.
-
Only users who can access the three-dot menu can view it.
